Dr.S. Bahram Mir Saeed Ghazi, Dr. Mojtaba Dibaee , Dr. Peyman Salamati , Dr.Ali Akbar Rahbarimanesh and Dr.Hamed Akhlaghi , evaluated 25 patients admitted to hospital with the diagnosis of adverse drug reaction between years 1998 to 2005 .the average age was 4.6(± 3.7)years and symptoms of adverse drug reactions were observed 12.6(±14.3)days after initiation of the drug theropy skin rash was seen in all patients and arthralgia was the next common symptom (44%)and the common abmormal laboratory data was high ESR which was seen in 40% of patients. The most ingested drug were phenothiazine(28%),sulfaslazine(28%),penicillin(16%),furazolidione(16%)cephalosporins (4%) and valproic acid (4%).In 28% of patients poly-pharmacy was responsible for ADR.this study demonstrated that awareness of the problem,observation of poly-pharmacy and potential drug reactions, and continious re-evaluation of the ongoing individual pharamacotherapy is important,especially in children,to reduce ADRs,
